An intraoral periapical (IOPA) radiograph was the first step in investigation. It revealed a well-defined, unilocular radiolucency surrounding the crown of the unerupted tooth 11. A panoramic radiograph confirmed the diagnosis, displaying a cystic lesion attached at the CEJ of the impacted central incisor—a textbook sign of a dentigerous cyst.
Surgical Treatment
A provisional diagnosis of a dentigerous cyst was made. Surgical enucleation was the chosen approach, coupled with extraction of the impacted tooth. The procedure was carried out under local anesthesia and completed uneventfully.
The excised specimen included the unerupted tooth enveloped by the cyst lining.
The intraoperative photo clearly shows the exposure of the cystic cavity in the anterior maxilla.
